Surgical Options for Cervical Degenerative Disc Disease in France: Fusion Versus Motion Preservation

Cervical degenerative disc disease significantly impairs quality of life, often causing neck pain, stiffness, and neurological symptoms. In France, where healthcare innovation intersects with patient accessibility, surgical treatments have diversified beyond traditional fusion. These techniques fall primarily into three categories:

• Anterior Cervical Discectomy and Fusion (ACDF): The conventional approach, involving removal of the damaged disc and fusion of adjacent vertebrae to eliminate motion at the affected segment.
• Cervical Disc Arthroplasty (CDA) or Motion-Preserving Disc Replacement: Implantation of an artificial disc designed to preserve segmental motion, thereby maintaining the natural biomechanics of the neck.
• Hybrid Techniques: Combining fusion and motion preservation for multi-level degenerative disease, tailored to patient-specific pathology.

The decision between fusion and prosthetic replacement hinges on patient pathology, age, and lifestyle demands. Fusion effectively stabilizes painful segments but sacrifices mobility, occasionally accelerating degeneration in adjacent segments. In contrast, prosthetic discs manufactured by leading companies—such as Prodisc, Mobi-C, and Prestige LP—are designed to maintain physiological motion, thereby potentially reducing secondary degeneration and preserving neck function.

Clinical studies supporting CDA demonstrate comparable or superior outcomes relative to ACDF, with many reporting lasting follow-up results over a decade. Advantages include faster recovery times, diminished postoperative stiffness, and maintenance of segmental kinematics. Yet, risks such as implant wear or mechanical failure necessitate careful patient selection and surgical expertise.

This range of options represents a continuum of care optimized through patient-specific evaluation. For Quebec patients exploring surgery abroad, France offers a robust ecosystem of expertise and technology, with facilities endorsing the latest regulatory approvals and safety standards.

Biomechanical and Clinical Advantages of Cervical Disc Prostheses Over Fusion

The biomechanical premise underlying cervical disc prosthesis is preservation of the natural motion segment, which plays a critical role in maintaining cervical spine mechanics and health. By avoiding the rigidity of fusion, these artificial discs aim to mitigate adjacent segment degeneration and reduce long-term complications.

Research comparing devices such as the Prodisc, Mobi-C, and Prestige LP discs has demonstrated that preserving segmental mobility positively impacts facets joints, kinematics, and load sharing. These benefits translate clinically into improved pain relief, range of motion, and patient satisfaction.

• Reduced adjacent level degeneration: Spinal fusion alters biomechanics, increasing load on neighboring discs.
• Maintained range of motion: Artificial discs allow flexion, extension, and rotation similar to natural discs.
• Lower revision surgery rates: Evidence shows fewer secondary procedures required with CDA compared to fusion.
• Faster functional recovery: Patients generally mobilize sooner and return to activities quicker.

According to meta-analyses and long-term cohort studies published through 2024, motion-preserving surgery with devices like Bryan Disc offers outcomes equal or superior to ACDF over 10 to 15 years. Besides biomechanical advantages, the procedure tends to reduce the risk of dysphagia and other anterior cervical complications.

The integration of modern materials combined with refined surgical techniques ensures that cervical disc prostheses achieve both durability and biomechanical fidelity.

Exploring and Evaluating Conservative Treatments Before Considering Surgery

One of the core principles guiding treatment evaluation, particularly emphasized by Dr. Desforges, is the necessity to exhaust conservative therapies before advancing to surgery. This approach aligns with scientific evidence and ethical care standards. The recommended treatment duration is typically 3 to 6 months, incorporating interventions tailored to patient symptoms.

Notable among advanced conservative therapies offered at the Clinique TAGMED is neurovertebral decompression, which aims to non-invasively alleviate nerve compression and promote natural healing. This procedure, using cutting-edge technology, complements physical therapies, pharmacologic pain management, and lifestyle modifications.

• Physical therapy: Targeted exercises to enhance spinal stability and mobility.
• Neurovertebral decompression: High-tech device-assisted treatment reducing disc pressure.
• Pharmacologic interventions: Anti-inflammatory and analgesic medications to control symptoms.
